Bringing Required Information

Online Registration

If you haven’t already, you can register as a new patient online. This can save you time and prevent forgetting to bring important information to your visit.

Please bring the following with you on your first visit:

  • Photo ID
  • Insurance cards
  • Referral card or slip
  • List of current medications
  • Medical and dental health history
  • Completed medical/dental insurance forms
  • Recent X-rays (within 6 months)

To ensure accurate diagnosis, it is very important that x-rays are recent (within six months). If you had x-rays taken with a previous dentist, you may request that they forward them to our office. If additional films seem necessary, they can be taken at our facility.

Your first visit to our office will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation.

However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.
